Beginning in May, Community Health Funding Week each month will honor specially selected community health service providers and/or individuals engaged in community health-related programs. Each month, our editorial board will review nominations from across the nation, selecting the top nominees and spotlighting their achievements for national recognition on the pages of CHF. And we want you, to be part of the selection process. If you know of an individual involved in community health services or a community health agency that's providing outstanding service, we want to hear from you.

The nomination process is informal and easy. Simply send us the name of the nominee, along with a little information - i.e., what, in your opinion, makes the nominee special? And if you can point us to a Web site, that would be very helpful.
