Point-Counterpoint: Use of ACE Inhibitors for Heart Failure in Blacks  

Point

Systolic heart failure (HF) is associated with a more rapid progression and a greater hospitalization and mortality rate in Black as compared to White patients. In general, therapy with an angiotensin converting enzyme inhibitor (ACE-I) is established to improve both the clinical course and mortality of HF.

Counterpoint

The SOLVD studies addressed systolic but not diastolic HF, a common cause of HF in Blacks. Current guidelines do not recommend ACE-I for either White or Black patients with diastolic HF (ie, diastolic dysfunction) because there are no available clinical trial data upon which to base therapeutic recommendations.

 

The ISHIB “Point-Counterpoint” group is represented by a rotating panel of 6-8 ISHIB members with expertise in cardiovascular disease and related cardiovascular risk factors. The purpose of the group is educational with a focus on addressing current clinical controversies in an attempt to provide a concise and balanced opinion for readers of the ISHIB Insider or the ISHIB journal, Ethnicity and Disease. The opinions expressed in “Point-Counterpoint” are not ISHIB consensus statements per se, although each report represents a summary by a minimum of 3 expert reviewers.

The Center for Continuing Medical Education at The Ohio State University Medical Center was founded in 1962. They are dedicated through a rich and innovative tradition to providing high quality continuing medical education to physicians and other healthcare professionals in order to facilitate the delivery of better OSUand enhanced patient care and outcomes.

The Ohio State University Medical Center is accredited by the Accreditation for Continuing Medical Education (ACCME) to sponsor continuing medical education for physicians, and designates CME educational activity on an hour-per-hour basis in Category 1 credit toward the American Medical Association (AMA) Physician’s Recognition Award. ISHIB Staff Participates in Reading ProgramEverybody Wins!

Two ISHIB staff members--Melanie Cockfield and Chris Fitzpatrick--donate 1 lunch hour a week to mentoring an at-risk elementary school student through the Everybody Wins! program. Everybody Wins! is a nonprofit organization dedicated to promoting children’s literacy through shared reading experiences with caring individuals. Their goal is to reach children while they are young, stimulate their interest in reading and create lifetime readers that will have better opportunities for success in school and life.

Founded in 1991, Everybody Wins! serves thousands of children nationwide with its Power Lunch reading program. The program targets public elementary school students who are at-risk for academic failure and enriches them with a mentoring experience designed to expand their horizons for success in school and in life.

Melanie and Chris have challenged other ISHIB staff to join them in this valuable program and prove that everybody wins when adults read with children. For more information visit www.everbodywins.org.
